A care plan is an easy, accessible way the needs of a person, what their preferences and choices are. It can include information relevant to all members of a person's care and health team, and resources on hand to meet that persons need.
When working with support professionals it can become a valuable resource to help guide your team on how to best support you in the home and accessing the community. A care plan can also act as a communication tool with your team and should be used as a living breathing document that evolves as your NDIS or aged care goals change.
